Step 1: Decide the Fields, in Priority Order

Now rank them. Due back is first, because that is the field people scan for across a shelf. Asset ID is second, because that is what ties the tag to your inventory record. Everything else is supporting detail that only matters once someone has already picked up the case.
Design to that ranking. Set the due-back date large, at 18 point or more, in a strong weight, ideally in a boxed area of its own. Set the asset ID at 14 point. Drop the remaining write-in fields to 9 or 10 point labels with lines sized for handwriting, which means a line spacing of 0.3 inches at minimum.
If you use a barcode or QR code, give it its own zone at a printed size of 0.75 inches or larger, with clear white space around it. Do not print it across a colored panel or a texture.
Step 2: Choose a Tag Format

Place the punch hole 0.25 inches in from the top edge, centered, with a hole diameter of 0.2 inches. Keep all text 0.4 inches clear of the hole, because the area around a punch is exactly where tags tear and ink rubs.
Step 3: Set Up the File at 300 DPI With Bleed
